WHAT IS WIC?

WIC stands for the Special Supplemental Nutrition Program for Women, Infants and Children. It's a USDA federally funded program that provides supplemental foods and nutrition education to income eligible pregnant, breastfeeding, postpartum women, and infants/children up to five years old.

WIC ELIGIBILITY

Residency: Must live in Kansas
Categorial: Must be pregnant, breastfeeding or postpartum woman - Infant or child up to five years old.
Nutritional Risk: Must demonstrate nutritional risk as determined by a health professional


HOW DO I APPLY?

You can get started by using the online WIC pre-application tool to apply for WIC services. Once your application is received the local WIC agency will contact you to set up an appointment. If you’re transferring from another state, please do not fill out the online pre-application. Instead, call the Leavenworth WIC office at (913) 250-2040.

At the appointment, there will be a brief review of foods you eat, medical history, and current health status to determine the medical or nutritional need. If you or your child is found eligible for WIC benefits, the appointment will also include a nutrition education session with one of the Registered Dieticians.

WIC Pre-Application

WHAT DO I NEED TO BRING FOR MY APPOINTMENT?

Proof of Identity:
1. Birth Certificate
2. Driver's License 
3. KS Medicaid Card 
4. Military ID
5. Passport
6. Photo ID
7. School ID

Proof of ID for Child:
1. Birth Certificate 
2. Foster Care/Adoption paperwork
3. KS Medicaid Card
4. Shot/Immunization records 

Proof of Residency:
1. Envelope that was mailed to you
2. Official letter
3. Rent receipts
4. Utility bill

Proof of Household Income for all people living in your house:
1. Income tax form for self-employment
2. Medicaid Title XIX card
3. Paycheck stub from current job

Food Packages

Pregnant, breastfeeding, postpartum women, and infants/children up to five years of age may receive:
Milk Natural cheese
Eggs Iron-fortified cereal 
Dry beans, peas, or peanut butter Canned beans
Tuna (as applicable) Pink salmon (as applicable)
Fresh fruits and vegetables 100% whole wheat bread
Tortillas Brown rice
Yogurt Tofu
Whole wheat pasta Vitamin C rich juice 
Infant cereal, baby food fruits, pureed meats and baby food vegetables Infant formula 

Special food packages are available for the homeless, children, women with special health needs, and women who exclusively breastfeed.
